Steps
1) Before conducting the outreach, jointly plan with the health teams and community leadership on the venue, date and time of the outreach and services to be provided. Target areas where facility coverage is low. Agree on number of days the outreach will last (should not exceed 3 days).
2) Map out the areas where the outreach will be conducted and identify the link facilities where referrals for other services can be made.
3) Clearly mark all the service delivery points (Arrival, Registration, Triage, Diagnosis and treatment, health-talk and ‘edutainment’ etc).
4) Ensure outreach package is available and checked by health team prior to departure
5) The outreach should be preceded by health talks and mobilization campaigns that can be done through community activities (through opinion leaders, religious leaders, etc) or by the PHC service providers during the household visits.
6) Ensure that there is privacy during service provision.
7) Ensure that everyone accessing a service is recorded in the respective reporting tools.
8) Information from the outreach should be consolidated in the MS1 at the end of the month and submitted to DPNO’s.
9) Hold debriefing session with community leaders and agree on the venue and date of next outreach.
Remember
Take into consideration the culture of the community when getting the teams to provide the services e.g. gender of service providers, age etc.
